California Privacy Notice (CCPA/CPRA)

This California Privacy Notice (“Notice”) supplements the Privacy Policy of Telezen MD, its owners, and affiliates (collectively, “Telezen MD,” “we,” “us,” or “our”) and applies solely to personal information collected from California residents (“consumers,” “you,” or “your”) in our capacity as a business under the California Consumer Privacy Act of 2018, as amended by the California Privacy Rights Act of 2020 (CCPA/CPRA).

Telezen MD does not sell personal information.

Telezen MD does not share personal information for cross-context behavioral advertising as defined under the CPRA.

We may disclose personal information to third parties for business purposes, including:

  • Licensed pharmacies and healthcare providers to fulfill prescriptions and provide care

7. How to Submit a Privacy Request

You may submit a request to exercise your privacy rights by:

You may designate an authorized agent to make a request on your behalf. We may require proof of authorization and may request verification directly from you.

You may submit a request to know, delete, or correct personal information no more than twice within a twelve (12) month period, as permitted by law.

We will confirm receipt of your request within ten (10) business days and respond within the timeframes required by applicable law. Our response will be provided in writing, unless otherwise requested or permitted.

We may update this California Privacy Notice from time to time. If we make material changes, we will update the “Last Updated” date above. We will not use personal information collected previously for materially different purposes without obtaining consent where required by law.
